Fees
· Call to discuss instructor's rates and participant fees for on-site presentation.
· Participant fees cover the cost of texts and course handouts.
· A certificate of participation documenting training is provided to each participant.
· Annual issue of ITEM included in packet materials.
· Suggested reading materials are included along with an order sheet for additional articles.
· Inclusion on mailing list to receive notification of regulation updates and articles.

· Purchase order is required to hold date.

Special Site Requirements
· Secure space that is large enough for everyone to see overhead slides.
· Access and assistance with set up and tear down - All audio-visual equipment provided by instructor.
